Student Agreement for a Personal Service Attendant (PSA):
- Submit a request for each class at least 2 business days before a PSA may be allowed to perform their tasks
- The PSA may only assist you with personal care, mobility, guidance and/or communication as stated on your Accommodations Form

Personal Service Attendant (PSA) Accommodation
It is required that the DSPS Counselor has approved the PSA as an accommodation for the current academic year. All paperwork must be submitted to DSPS prior to the PSA accompanying the student. The approval process may take up to 5 business days. If the PSA has not been approved please contact the DSPS office at 949-451-5630 or ivcdsps@ivc.edu.

Student Agreement for Transcripts and Recording
- The student may not copy or release any transcripts and recording material received or otherwise hinder the ability of the professor(s) to obtain a copyright on class material.
- If staff has reasonable grounds to believe the transcripts and/or recordings are being used in violation of this agreement, it may result in disciplinary action and/or the deactivation of the license.
- The service will automatically terminate on the last day of finals.
Volunteer Note Taker Guidelines
Thank you for volunteering to take notes this semester! Upon confirmation of services rendered, DSPS will provide you with a letter of commendation from the Dean of Counseling Services for your invaluable service.
